Please respond with your information filled in below as soon as possible. Once we have your information, we will reach out to get your first visit scheduled.

Please forward a digital copy of your pet’s medical record to us (you can also take photos of those pages and email them to us as attachments or upload them in the form below) before your visit.

If your pet has been seen by another veterinarian, please give them a call and have them fax or email your pet’s medical records and lab work results to us at: Fax- (617) 244-1697 or Email- [email protected]

Having this information in hand allows us to update your pet’s file before you arrive, and have a plan in place at the time of your appointment.

Please familiarize yourself with CVH’s policies prior to your appointment:

• For their safety, all pets are required to be on a leash or in a carrier at all times.
• All pets will, at all times, be treated with the care and compassion with which we would treat our own family members.
• Photos of pets may be taken while in the practice, and may occasionally be used for educational purposes (ex.: a picture of an unusual lesion might be used in a lecture presentation), or posted on social media (ex.: people love cute photos of your pets). No images or information will be used that would identify you personally without your permission.
• I understand that CVH does not submit claims to pet insurance companies. I understand that CVH will provide me a copy of invoices and any needed information for me to submit claims. I understand that payment in full is due at the time of my pet’s visit.
